CONTACT US: 3600 Main Avenue, Suite C Durango, CO 81301 970.259.7171 cottonwoodholistic@gmail.com
OFFICE HOURS: Our office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday to address your health and wellness.
APPOINTMENTS: Appointments are best made in advance and we work hard to make same day appointments available to our established patients. We are located on North Main Avenue in Durango, CO.
INSURANCE: We accept most private insurances and see clients without insurance as well. Contract obligations with insurance companies require us to collect co-pays at the time of service.
SELF-PAYING PATIENTS: We see many patients who do not have insurance. We offer discounted services for prompt payment. Patients without insurance are asked to pay for services provided upon checkout that same day.
ON-CALL AND AFTER HOURS CARE: After hours, weekends and holidays, please call the office phone for instructions to reach the on-call physician. We tend to be on-call for ourselves most of the time, but will occasionally arrange for coverage from another doctor in the area.
HOSPITAL CARE: As your primary care physician, we specialize in office-based family medicine. We partner with hospitalists (physicians specializing in hospital care) at Mercy Medical Center who manage the care of our patients when they are hospitalized. We work closely with them to share information, plans of care and follow up needs.
PRESCRIPTION MEDICATIONS AND REFILLS: We believe the appropriate use of prescription medications can add to a person’s quality of life and can be essential in treating and preventing a variety of illnesses. Appropriate use of medication requires a complete evaluation of the risks and benefits of medications for each individual situation. Medications we have not prescribed for a person in the past cannot be filled without an office visit. Patients using medication on a regular or semi-regular basis require at least one visit in the office per year to review medication usage and current health status.
INTEGRATIVE HEALTH: We also support evidence-based alternatives in healthcare. We handle traditional medical needs as well as manage complementary methods.
